What to Bring
No matter the reason for your visit, always bring the following with you:
- Your driver’s license
- Your insurance ID card, if you have one
- Your medication bottles, along with any supplements and vitamins you take
You are welcome to bring the following for your hospital stay:
- An iPad, laptop or personal electronic device (wireless)
- Toiletries, make-up, etc.
- Personal comfort items, like books, puzzles, blankets, stuffed animals, etc.
Amenities
All patient rooms are equipped with cable television and telephones for outside calls. To make a local call, press “9” and dial the appropriate number. To make a long distance call, press “9-1,” followed by the area code and seven-digit number.
Personal cell phones may be used unless advised otherwise by your health care team.
An iPad is also available for hospital patients’ use. If interested, ask a nurse if it is available for use.
Free Wi-Fi Access
Free wireless internet access is available to patients and guests. To use wireless, please ask a nurse for the access code.
Interpreting Services
You have a right to a medical interpreter at no cost to you. Please let us know when you are scheduling an appointment if you would like to have an interpreter service available during your visit.

Health Care Directives
A health care directive is a written document that informs others of your health care wishes if you are unable to communicate them yourself.
Having a plan in place can save your family the stress and fear of having to decide what you’d want during a time of illness or injury.
Health care directives are available at Sleepy Eye Medical Center and can be downloaded by clicking on the link below. Our staff is available to answer your questions and provide helpful tools to assist you in completing a health care directive in accordance to Minnesota state laws.

Sleepy Eye Medical Center’s Health Information Management Department maintains and protects patients’ health records.
If you’ve been receiving care outside of Sleepy Eye Medical Center, you’ll need to complete an authorization form. This form allows outside facilities to share your medical record with us. Documentation of any previous health conditions and treatments will help our providers appropriately care for your health care needs.
You’ll need to complete this form if you plan to receive care outside of Sleepy Eye Medical Center as well.
If you send your spouse, family member, friend or significant other to pick up copies of your health information, you will need to indicate written permission for that individual on the form prior to their picking up this information.
Please note that if you’re requesting your medical records for personal use, you may be charged a fee.

Visitors
Sleepy Eye Medical Center welcomes family, friends and other guests to visit you during your stay. If you would like the support of a loved one during the night, please talk with your nurse.
Clergy are welcome to visit anyone at any time, with patient and/or family permission.
To protect the health and safety of all patients, we ask visitors to follow these guidelines:
- Sign in at the Nurses Station.
- Refrain from visiting patients if you have a respiratory infection or contagious illness.
- Children should be accompanied by an adult at all times.
- Check with a nurse before giving any food or beverages to a patient.
- Only latex-free (Mylar) balloons, please.
- Avoid sitting or lying on a patient’s bed.
- Refrain from smoking on campus. Sleepy Eye Medical Center is tobacco free.

At Sleepy Eye Medical Center, patient safety is our priority. We have a team of dedicated professionals, known as the Patient Safety Team, who work diligently to promote a culture of safety for our patients, visitors and employees. We support and participate in national safety efforts, programs and campaigns to ensure that our campus is a safe place for all.
Compliance Hotline
The compliance hotline is designed to allow patients, family members, employees and other individuals to confidentially and anonymously report any billing, coding or practice they believe is not in compliance with government regulations or Sleepy Eye Medical Center policy.
If you believe you have identified a potentially inaccurate or inappropriate billing, coding or practice, please call our compliance hotline at 507-794-8441. If you prefer, you may submit your information by email to [email protected]. Please note that your identity may become known to the compliance officer if you choose to email. However, your information will remain confidential.
