If you or your children fit the above criteria and would like to be added to our COVID-19 vaccine list, we offer two different options to sign up:
1. Complete our online sign-up form or
2. Call us at 507-794-8453 and leave a message with your 1) legal first and last name, 2) date of birth, 3) telephone number and 4) qualifying factor(s).
From there, we will contact you for scheduling. We encourage you to return to this page regularly for news and updates. Vaccine updates will also be shared on the SEMC Facebookpage.

Minnesotans who are unable to sign up online can call 833-431-2053 to sign up over the phone. Translation services are available.
