Getting Started with My SEMC Chart
How do I create My SEMC Chart account?
You can create your My SEMC Chart in four ways:
- Enroll yourself by following these instructions:
- Click on the blue "Visit My SEMC Chart" button at the top of this page or here.
- Click "Sign Up."
- Complete the enrollment request. (Note: This will only work if you have provided an email address to SEMC at registration.)
- Access your email and click on the link to the portal.
- Select a new user name and password.
- Visit the Apple or Google Play stores and download the MHealth app.
- Explore the portal.
- Call 507-794-3691 to talk with an online support specialist Monday through Friday, 8 a.m. to 5 p.m.
- Email our online support specialist at [email protected]. You will receive a response within 2 business days.
- Enroll at your next appointment, or by stopping by the clinic registration desk Monday through Friday, 8 a.m. to 5 p.m.
How do I get technical assistance?
Technical assistance is available for your My SEMC Chart by phone and email:

- Email our online support specialist at [email protected]. You will receive a response within 2 business days. Please note that we cannot answer medical questions, take appointments or refill prescriptions by email.
What are the username and password requirements for an account?
Your username must contain:
- 4-12 characters
- Only letters and numbers
Your password:
- Must be at least 6 characters
- Can be all lowercase, uppercase or a mixture of the two
- Must contain one symbol
- Must be different than your username
Can I access My SEMC Chart from a mobile device (tablet, smartphone, etc.)?
Yes, you can access the information found in your My SEMC Chart from a mobile device. Simply type www.semedicalcenter.org in your web browser, click on My SEMC Chart and sign in under My Account.
Proxy Access: Gaining Access to Another Person’s My SEMC Chart
Can I gain access to another person’s health record?
Proxy access allows you to share your health information or gain access to another person’s health information through your My SEMC Chart account. Proxy access is especially helpful if you manage the care of a child or another adult.
When you request proxy access to a child’s or adult’s interactive health record, you’ll have the ability to access many of the same online features you do with your own account.

Can I schedule appointments online?
Currently, we do not have online appointment scheduling.
Communication Preferences
How do I update my contact information online?
You can update your contact information under the account settings.
- Sign in to your account.
- Click on Settings
- In the Persona Information section, you can change your email address, street address and phone numbers.
Health Information
What can I do if I don’t understand my health information?
If you do not understand the health information on your account, you can:
- Call the clinic at 507-794-3691 and ask clarification
- Discuss your questions with your provider during your next clinic visit
What should I do if my health information is incorrect?
If you believe the information in your health record is incorrect, please call your provider to discuss your concerns.
Why don’t I see all of my lab results in my account?
Lab results for most tests are automatically released to your My SEMC Chart 72 hours after an appointment.
