Healthcare Information Privacy

Protecting your private health information is important to us. It may be necessary for you to go to the hospital or clinic to pick up copies of your health record. To help ensure confidentiality of your health information, you may be asked to sign an “Authorization to Release Medical Information” and be asked for picture identification. This will help us verify that copies are being given to the right individual.

If you send your spouse, significant other, child, in-law, friend or neighbor to pick up copies of your health information, we need a signed “Authorization to Release Medical Information” form signed by you indicating written permission for your family, neighbor or friend to pick up copies of your health care information, prior to their picking up this information. Forms can be obtained from the Health Information Management Department. The individual picking up your health information should also have picture identification.

Fall Prevention/Restraints

It is a goal of the Sleepy Eye Medical Center to prevent in-hospital injuries. Occasionally, restraints may be needed in order to protect a patient from injury to self or others. When restraints are necessary you can be assured we will use the least restrictive device possible.

Safety Program

The Sleepy Eye Medical Center has a program that focuses on patient, visitor, and employee safety. Safety is number one and we support national safety efforts such as awareness programs, national campaigns, confidential error reporting systems and patient education efforts, working diligently to promote a culture of patient, employee and visitor safety.

Infection Control nl

The Infection Control Committee for the Sleepy Eye Medical Center focuses on the prevention of the spread of infection throughout the medical center. We know the most effective way to prevent the spread of infection is through hand washing measures. We use cleaning products that are effective against many germs. We use special precautions with patients that are diagnosed with certain infectious diseases. We also report specific infectious disease findings to the Minnesota Department of Health as required by law.

Discharge Planning nl

Discharge planning arranges for your post-hospital care. You play an important role in the process. Your participation can help you –

  • learn what to expect from your recovery at home
  • get needed services to aid your recovery at home
  • understand your self-care instructions so you have a safe and smooth recovery

The Sleepy Eye Medical Center Discharge Planner will work with you to plan the best possible post-hospital care.

Healthcare Directives nl

Minnesota law allows you to inform others of your health care wishes. You have the right to state your wishes or appoint an agent in writing so that others will know what you want if you can’t tell them because of illness or injury. Interpreters and Hearing Services nl

Certified medical interpreters and services for the hearing impaired are availalble for our patients at no charge. Please let us know when you are scheduling an appointment if you need these services.

Credit Procedures nl

If you have Medicare, Medical Assistance, or other health insurance, please be sure that we have a current copy of your insurance card. Any charges not paid by your coverage will be billed to you and due within 30 days of receiving your statement. Checks returned to us for non-payment are subject to a $30 service charge in accordance with Minnesota Statute 332.50.

Compliance Hotline nl

The Compliance Hotline is designed to allow patients, family members, employees and other concerned individuals to report in a confidential and anonymous manner any billing, coding or other practice they believe is not in compliance with government regulations or Sleepy Eye Medical Center policy.
